Report of Disciplinary or Administrative Action	5/6/04	11/8/04	DOA	Incident date: 6/10/03 Investigation into a solider accused of robbery and malingering when as a member of a patrol team, he stole 180,000 Iraqi Dinar from two homes he was searching and then shot himself in the foot after being informed that he would be recommended for disciplinary action for stealing money. Soldier received an Other Than Honorable Discharge from the Miltary.
Report of Disciplinary or Administrative Action, Part 1, Part 2	1/27/04	11/8/04	DOA	Incident date: 6/1/03 – 7/8/03 Investigation into charges of 7 soldiers accused hitting an Iraqi detainee in the head several times, fracturing his jaw, shooting at vehicles and dogs in a traffic, and the taking of captured weapons in Baghdad, Iraq. Investigation resulted in 5 guilty charges, and 2 dismissals of charges: failure to obey general order, failure to obey direct order, failure to turn over captured property, and failure to turn in seized property by order of the commander. Punishment of a reduction in rank and pay deductions.

Report of Disciplinary or Administrative Action	10/1/03	11/8/04	DOA	Incident date: 8/31/02 Investigation of assault by soldier who threatened to kill Iraqi civilians in custody of US forces in an army compound in Baghdad, Iraq. Soldier admitted to having no formal training as an interrogator and was not aware of a formal army policy on interrogation techniques. Investigation terminated. If an action commander or prosecutor indicates either to take no action against the subject or take action amounting to less than a court proceeding and no further investigative assistance of CID is required by the commander or prosecutor.
Report of Disciplinary or Administrative Action	8/27/04	11/8/04	DOA	Incident date: 11/7/02 Investigation into soldier who had given his wife several compact disks containing images depicting the possible abuse of detainees. Army found the CD revealed pictures of detainees from Afghanistan, but none of the detainees are being abused or mistreated. Investigation terminated in that the Special Agent in Charge of this office has determined that the furtherance of this investigation would be of little or no value and lead remaining to be developed are not significant.
Report of Disciplinary or Administrative Action	7/15/04	11/8/04	DOA	Incident date: 1/11/04 – 1/19/04 Investigation into a recordable compact disc containing images of suspected detainee abuse in unknown locations in Afghanistan. This investigation was transferred to the 62^nd MP det (CID) on 7/13/04.
Report of Disciplinary or Administrative Action	7/16/04	11/8/04	DOA	Incident date: 4/1/03 – 7/31/03 Investigation into a soldier accused of stealing prayer beads and other personal items from prisoners at Guantanamo Bay. Also had several CDs of photos taken while at Guantanamo. Investigation terminated – furtherance of investigation would be of little or no value.
Report of Disciplinary or Administrative Action	7/14/04	11/8/04	DOA	Incident date: 2/5/04 Investigation into a solider accused of maltreatment of a prisoner and assault when he struck a detainee during an interrogation at Camp Bucca, Iraq. Soldier was reprimanded, received a reduction in rank, and pay was taken away.
Report of Disciplinary or Administrative Action	8/27/04	11/8/04	DOA	Incident date: 7/12/03 Soldiers accused of housebreaking and larceny in Camp Anaconda, Balad, Iraq. All three soldiers received a reduction in rank, and pay was taken away.
Report of Disciplinary or Administrative Action	1/30/04	11/8/04	DOA	Incident date: 12/2/03, 12/12/03, 12/13/03 Investigation of a soldier in possession of valium and stolen money from an Iraqi in Baghdad, Iraq. Investigation complete – prosecutor believes he has sufficient evidence to prosecute perpetrator for offenses.
Report of Disciplinary or Administrative Action, Part 1, Part 2, Part 3	8/17/04	11/8/04	DOA	Incident date: 11/3/03 Investigation into allegations of detainee abuse at Abu Ghraib in Sept. 2003 by members of the 519th Military Intelligence Batallion. SPC at Abu Ghraib provided sworn statement that he overheard a MP dog handler stating that a game was being played to see which dog handler could get the most detainees to urinate on themselves. Investigation established the offense of detainee abuse did not occur as alleged by members of the 229th MP company.
Report of Disciplinary or Administrative Action, Part 1, Part 2, Part 3	9/13/03	11/8/04	DOA	Incident date: 9/1/03 Investigation of abuse of detainees at Camp Muleskinner, Baghdad, Iraq. (maltreatment of prisoners, assault, false swearing, accessory after the fact, obstruction of justice, and dereliction of duty). Soldiers later made an audiotape summarizing events involving the beatings of detainees. All 5 soliders accused of abuse were found guilty of maltreatment of prisoners, assault, accessory after the fact, obstruction of justice and dereliction of duty. Punishment: reduction in rank and reprimand.
Report of Disciplinary or Administrative Action	8/16/03	11/8/04	DOA	Incident date: 6/21/03 Investigation into an incident of two soldiers assaulting an Iraqi civilian who had been taken into custody of the US Army – near Iraq Museum of Military History, Baghdad, Iraq. Investigation established probable cause to believe that the two soldiers committed the offense of assault and dereliction of duty when they assaulted an Iraqi citizen. Acts included forcing detainee to hold a pistol so soldier could have excuse for shooting detainee. Only one of the soldiers was found guilty and given a reduction in rank.
Report of Disciplinary or Administrative Action	6/24/03	11/8/04	DOA	Incident date: 4/18/03 Investigation into the death of Yasiree Ahmes Al-Haddii (Enemy Prisoner of War) at Camp Bucca, Um Qasr, Iraq. Yasiree committed the offenses of attempted murder, rioting, and aggravated assault. Two U.S. soldiers were ordered to fire on Mr. Yasiree. Mr. Yasiree died as a result of being shot by the soldiers. Investigation revealed that both soldiers did not commit any offense as they were ordered to fire on Mr. Yasiree and were within the Rules of Engagement.
Report of Disciplinary or Administrative Action	11/23/03	11/8/04	DOA	Incident date: 9/11/03 Investigation into the death of detainee Obeed Hethere Radad at Camp Iron Horse, Tikrit, Iraq. Specialist from the 4th Fwd. Support. Bn., 4ID, shot and killed Radad, without first issuing warnings as required under the Rules of Engagement. CID did not receive a report of the incident until 9/16/03. Due to the delay, no autopsy; crime scene significantly altered; and evidence not collected. CID determined that probable cause existed for a murder charge. However, the day after the incident, Army held an Art. 32 hearing charging the Specialist with Voluntary Manslaughter (1057). Specialist reduced in rank and discharged from the service, before the investigation had even begun.

Report of Disciplinary or Administrative Action	7/14/04	11/8/04	DOA	Incident date: 2/7/04 Investigation into cause of death of detainee at Division Central Collection Facility (DCCF), Tikrit, Iraq. Determined cause of death was natural as a result of heart disease.
Report of Disciplinary or Administrative Action	10/2/04	11/8/04	DOA	Incident date: 6/12/03 Investigation into the attempted escape of two detainees and the shooting death of one of the escapees at Camp Cropper, Iraq. Ruled: Justifiable Homicide.
Report of Disciplinary or Administrative Action	5/19/03	11/8/04	DOA	Incident date: 5/12/03 Investigation into a fatal shooting of a civilian by a tower guard at Camp Bucca International Facility, Iraq. Conclusion of investigation determined that soldier failed to follow prescribed Rules of Engagement detailing the use of warning shots to deter individuals from entering the camp. Soldier received a reprimand for his actions.
Report of Disciplinary or Administrative Action	7/22/04	11/8/04	DOA	Incident date: 4/19/04 Investigation into undetermined manner of death of detainee at Abu Ghraib prison in Iraq. No crime scene exam was conducted, no autopsy conducted, no copy of medical file obtained for investigation because copy machine broken in medical office.
Report of Disciplinary or Administrative Action in the case of Mr Abdul Kareem, Part 1, Part 2	1/1/04	11/8/04	DOA	Incident date: 12/9/03 Investigation into undetermined manner of death of detainee being held in Mosul, Iraq (in a military police battalion). No autopsy was taken, detainee has no known medical history, nor had he requested medical assistance prior to his death. Investigation could not determine cause of death without the benefit of a forensic autopsy.
Report of Disciplinary or Administrative Action	2/11/04	11/8/04	DOA	Incident date: 11/22/01 Investigation into an attempted murder of U.S. soldiers by a detainee at Abu Ghraib prison in Iraq. Investigation concluded that there is enough admissible evidence to prosecute the subjects.
Report of Disciplinary or Administrative Action Part1, Part 2, Part 3, Part 4, Part 5	10/10/03	11/8/04	DOA	Incident date: 5/25/03 Investigation into soldiers stealing money while conducting a raid of Iraqi residence off Highway 1 in Bayji, Iraq. Investigation terminated because there is sufficient admissible evidence to prosecute the subject for the offense.

Report of Disciplinary or Administrative Action	8/24/04	11/8/04	DOA	Incident date: 4/10/04 Investigation into incident of rioting during which a detainee was shot and killed by Marines at Abu Ghraib prison, Iraq. Investigation established probable cause to believe that detainees committed the offense of rioting. The shooting of both detainees was justifiable and no criminal offense was committed by either soldier.

Investigation determined there was not sufficient evidence to prove or disprove the allegations. Investigation terminated.
Report of Disciplinary or Administrative Action	7/22/04	11/8/04	DOA	Incident date: 1/1/04 Investigation into maltreatment of a prisoner at Ad Diwaniyah Military Correction Facility, Iraq. Soldier was shown a picture of a naked female Iraqi prisoner on the computer while in Iraq. Investigation found insufficient evidence to charge anyone with maltreatment of a prisoner. Investigation established insufficient evidence to believe soldier committed the offense of maltreatment of prisoners.
Report of Disciplinary or Administrative Action	2/4/04	11/8/04	DOA	Incident date: 8/3/03 Investigation into death of an Iraqi detainee at Camp Cropper, Iraq. All investigative efforts to confirm the victim was transported to the US or Germany for an autopsy have met with negative results. No record exists as to whom the body was released to. Manner of death ruled: natural causes.

Investigation established that soldier did not commit the offense of Assault based on the Rules of Engagement that allow a soldier to use appropriate force to detain a person thought to be involved in terrorist acts.

Report of Disciplinary or Administrative Action, Part 1, Part 2	8/7/04	11/8/04	DOA	Incident date: 11/24/03 Investigation into the shooting deaths of 4 detainees at Abu Ghraib prison, Iraq. detainees participated in an organized, violent uprising within the prison. Investigation concluded that the deaths resulted in justifiable homicide and no criminal act was involved.

Investigation established that the shooting of both detainees was justifiable and neither soldier committed criminal offenses.

Report of Disciplinary or Administrative Action	2/10/04	11/8/04	DOA	Incident date: 5/12/04 Investigation into cruelty and maltreatment and aggravated assault of 44 Enemy Prisoners of War by 10 US soldiers who were escorting the prisoners to Camp Bucca, Um Qasr, Iraq. 4 of the 10 soldiers investigated had administrative action taken against them. All 4 received a reduction in rank and an administrative discharge.

Report of Disciplinary or Administrative Action	7/1/03	11/8/04	DOA	Incident date: 6/22/03 Investigation into accusations of assault, conspiracy, willful dereliction of duty and cruelty and maltreatment when soldiers on two separate occasions conducted a mock execution of an Iraqi teenager. Location was near an ammunition factory in Gahzalia, Iraq. Investigation established probable cause to believe that solider committed assault, conspiracy, release of a detainee without proper authorization, willful dereliction of duty and cruelty and maltreatment. No records of results of prosecution of soldiers involved.

Unable to make any conclusions about investigation – no records to reflect that the man was ever detained by US forces.
